Atlantic Southeast CRJ2 at Baton Rouge on Sep 1st 2011, left main gear up landing

An Atlantic Southeast Airlines Canadair CRJ-200 on behalf of Delta Airlines, registration N875AS performing flight EV-5058/DL-5058 from Atlanta,GA to Baton Rouge,LA (USA) with 50 passengers and 3 crew, was on approach to Baton Rouge when the crew received an unsafe indication for the left main gear. The crew aborted the approach and entered a holding to troubleshoot, however, were unable to lower the left main gear forcing the crew to land on the right main and nose gear as well as left wing tip. The airplane came to a stop on the centerline of the runway, no injuries occurred.

The FAA reported the aircraft received substantial damage.

The NTSB reported on Sep 16th that the crew observed a landing gear disagree message while configuring the aircraft for landing and saw the left main gear was not down and locked. Despite actioning the relevant checklists including selecting the gear up and down again and the use of alternate gear extension the left main gear still did not extend. The crew elected to land on right main gear, nose gear and left wing, following the landing the airplane was evacuated through the left forward door. The NTB continued: "The airplane was lifted and the left landing gear extended." Further examination is being conducted.

On Jan 31st 2014 the NTSB released their final report concluding the probable cause of the accident was:

The improperly installed upper attachment bolt in the left main landing gear uplock assembly, which led to the failure of the left main landing gear to extend before landing. Contributing to the accident was maintenance personnel's lack of training on the installation and inspection of the uplock assembly.

The NTSB reported that maintenance had been done to the uplock assembly by two mechanics who had no prior experience to perform the installation of the uplock assembly. The airline changed their maintenance requirements to prevent future accidents. The aircraft manufacturer issued a service bulletin to address the maintenance discrepancy and avoid future accidents.
