GOL B738 near Sao Paulo on Oct 16th 2011, partial loss of instruments
On Jan 14th 2014 Brazil's CENIPA released their final post to public, that had been created on Dec 16th 2013 and that stated that an investigation into the serious incident involving the failure of system components has not being conducted - consequently no preliminary, interim, factual or final report has been released -, however safety recommendations in Portugese were released that permit some insight into the serious incident.
According to the brief abstract released with the safety recommendations the aircraft was accelerating for takeoff from Congonhas Airport nearing Vr (speed to rotate) when the master caution illuminated, both electronic engine control units switched to alternate mode and the IAS disagree indication illuminated. Immediately after the landing gear was retracted the indications for airspeed and altitude became significantly different, the stick shaker activated. The crew managed to control the aircraft and diverted to Viracopos Airport for a safe landing.
The safety recommendations are all directed at Brazil's ANAC (Civil Aviation Authority) recommending:
- evaluate the training programmes of commercial operators to have most critical emergencies trained more frequently during simulator exercises
- determine whether commercial operators should revise their procedures to accept new aircraft especially with respect to identify needs and empower their staff to maintain technological improvements present on new aircraft purchased
- determine definitive procedures to preserve cockpit voice recorder recordings in case of an aviation occurrence
- determine standard procedures for commercial operators to ensure that maintenance staff is in proper continuous contact/exchange of information with the Aviation Directorate of Operatinal Safety and are properly trained
- "disclose the contents of this report" (editorial puzzled question: which report?) for seminars, lectures and related activities focussed on commercial operators.
