Volga-Dnepr A124 near Ulyanovsk on Oct 28th 2014, engine shut down in flight

A Volga Dnepr Antonov An-124-100, registration RA-82046 performing freight flight VI-4340 from Moscow (Russia) to Baikonur (Kazakhstan) with 20 crew and freight consisting of satellite Astra 2G, was climbing out of Moscow near Ulyanovsk (Russia) when the crew decided to divert to Ulyanovsk due to the failure and inflight shut down of the the right outboard engine (D18T, #4). The aircraft landed safely.

The airline stressed the cargo did not get damaged, the crew decided to exercise extra precaution due to the value of the cargo and did not continue to Kazakhstan although the aircraft could have carried on on three engines.

Volga's Investigative Department reported the aircraft diverted to Ulyanovsk due to a problem with one of the engines according to preliminary data, the department is investigating the occurrence.
