Sriwijaya B735 at Padang on Mar 27th 2013, runway excursion

Indonesia's National Transportation Safety Committee (NTSC, also known as KNKT) released their final report concluding:

The Pilot in Command had partially missed of the 3 levels of the Situational Awareness and most likely contributed to the disobeying in some limitations of ground manoeuvring.

The NTSC reported that the captain (54, ATPL, 28,000 hours total, 5,420 hours on type) as assisted by a first officer (24, CPL, 600 hours total, 400 hours on type). The captain was feeling unwell with stomach ache on the preceding sectors with multiple visits to the toilet.

The aircraft had arrived into Padang and was taxiing faster than normal, according to ground witnesses, to the gate, the NTSB analysed at about 18 knots. The marshaller gave his stop command, however the aircraft continued and stopped only 4 meters past the stop position. The aircraft needed to be pushed back into position to be able to have passengers disembark normally.

The crew subsequently prepared for the onward sector to Jakarta. The captain was assigned pilot flying for that sector.

The aircraft taxied out and backtracked runway 15 to line up at the threshold. The aircraft was turned to the left but stopped at a heading of 270 degrees with all wheels on soft ground off the runway.

All occupants disembarked safely through the aft door. The aircraft was moved to the apron the following day. The aircraft sustained no damage.

The NTSB analysed:

Three significants condition of the pilot associate to the situational awareness are:

- The PIC was claimed that he had stomach ache and did not reported to the company.

- The PIC was late to recognise the marshaller (AMC officer) sign to stop the aircraft during parking.

- The PIC made a turn while the aircraft speed was 11 knots higher than the recommended speed started from the centre of the runway.

Refer to the those conditions it can be concluded that the Pilot in Command had partially missed of the 3 levels of the Situational Awareness and it most likely contributed to the disobeying in some of limitations of ground manoeuvring.
